• Respond to referrals, perform financial assessments, and facilitate post-discharge follow-up to support patient outcomes.
• Ensure accurate documentation in electronic health records and monitor compliance with departmental standards.
• Lead, coach, and mentor team members, participate in hiring, training, and performance evaluations.
• Support process improvement initiatives and contribute to quality and safety activities.
• Coordinate coverage, assist with scheduling, and facilitate team communication.

Qualifications & Skills:

• Minimum of 3 years recent care coordination experience in a hospital setting, with exposure to diverse medical populations preferred.
