Job summary

Civic Center Health and Rehabilitation is seeking a skilled Registered Nurse (RN) to provide direct nursing care to residents in our long-term care and rehabilitation facility.

You will assess residents’ health, administer medications, and collaborate with the clinical team to ensure high-quality, compassionate care. BLS/CPR is required. Shift is 3p–11p; this is a full-time role with competitive benefits and a $12,000 sign-on.

Qualifications

• Valid RN license
• Strong knowledge of geriatric and rehabilitation care

Responsibilities

• Deliver patient-centered nursing care to residents in a long-term care and rehabilitation setting.
• Assess, monitor, and document residents’ health conditions and progress.
• Administer medications and treatments as prescribed, ensuring compliance with facility protocols.
• Collaborate with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans.
• Provide education and support to residents and their families regarding care plans and health conditions.
• Maintain accurate and timely documentation in compliance with state and federal regulations.
• Foster a safe, comfortable, and therapeutic environment for residents.
• Supervise and mentor nursing assistants to ensure quality care delivery.
