Job summary

Trinity Health Of New England At Home (Connecticut) is seeking a Home Health Palliative Care RN to deliver compassionate, high-quality nursing care in patients’ homes in the Enfield region.

You will provide palliative care consultations, develop care plans with the Population Health Case Manager, and supervise Home Healthcare Aides and LPNs. A 10k sign-on bonus applies for this full-time role with a Mon-Fri 8am-5pm schedule and weekend rotation every 4 weeks.

Qualifications

• Graduate of an approved nursing education program.
• Licensure as a Connecticut Registered Nurse (RN).
• One year experience as a professional acute care nurse.
• Home care experience preferred but not required.
• Must have current Driver’s license and reliable transportation.

Responsibilities

• Provide palliative care clinical consultation.
• Provide primary nursing care to home-based care patients as ordered by the physician.
• Develop initial assessment and care plan with Population Health Case Manager.

The Palliative Care RN is responsible for patients and families in the palliative care program. Provides Palliative care clinical consultation. Provides primary nursing care to home based care patients as ordered by the physician, and in accordance to the organization’s policies and procedures. The RN is responsible for the initial assessment and care plan development in collaboration with the Population Health Case Manager. Provides functional support/supervision to Home Healthcare Aides and LPNs as needed. Provides therapeutic intervention, overall care management and achievement of top decile client outcomes and patient satisfaction.
