Job summary

Trinity Health Of New England At Home (Connecticut) seeks a Home Health RN to deliver skilled nursing care in patients’ homes within the West Hartford region. You will develop individualized care plans, administer medications, and educate patients and families to support recovery and independence.

Experience in home health is preferred, but transition from hospital or acute care is welcomed. The role offers a Monday–Friday schedule, weekend rotation every 4 weeks, and a $10,000 sign-on bonus.

Qualifications

• Graduate of an approved nursing education program.
• One year experience as a professional acute care nurse.
• Home care experience preferred but not required.
• Must have current Driver’s license and reliable transportation.

Location: West Hartford Region

Status: Mon – Fri 8 am to 5 pm, weekend rotation every 4 weeks, 2 holidays

• Graduate of an approved nursing education program, BSN preferred.
• Licensure as a Registered Nurse in the State of Connecticut.
• One (1) year experience as a professional acute care nurse
