Job summary

Brighton Hospice is seeking an experienced Registered Nurse (RN) to provide compassionate in-home and facility-based care to a group of patients within our Oregon service area. You will collaborate with an interdisciplinary team to deliver psychosocial assessments and clinical interventions.

Travel is required across the service area; a valid driver's license, reliable transportation, and ability to learn our EMR system are essential.

Qualifications

• Current RN licensure (in good standing) in state(s) of practice.
• Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Work closely with patients on a 1-1 level but get strong support from a patient care team.
• Can work independently and on your own from a day-to-day basis with activities including phone, email and written communication, meetings with healthcare professionals, and understanding hospice rules and regulations.
• Time management skills to manage visits, documentation, meetings and urgent needs by prioritizing and triaging multiple concerns.
• Ability to learn an electronic medical records system.
• Must be willing to drive with reliable transportation, valid driver’s license, and auto insurance to travel across the service area.

Responsibilities

• Provide care and services for a group of patients, visiting patients where they reside (homes, SNFs, assisted living).
• Be a contributing member of the interdisciplinary team, offering expertise in psychosocial assessments and community resources.
• Assume Case Management, Admissions, or Resource responsibilities as part of the team.

The Registered Nurse (RN) will be responsible for providing care and services for a group of patients, visiting patients where they reside, which could include private homes, Skilled Nursing Facilities, and assisted living facilities. This position is a contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources. The Registered Nurse (RN) will work in either a Case Management, Admissions, or Resource role.
