Grant Healthcare and Rehabilitation seeks an RN in Kentucky to join our long‑term care team. You will coordinate care plans, administer medications, and communicate with patients to ensure compassionate, high‑quality service.

RN Core Responsibilities:

Adherence to standards of business conduct and compliance
Consults and coordinates with healthcare team members to assess, plan, implement and evaluate patient care plans
Prepares and administers (orally, subcutaneously, through an IV) and records prescribed medications. Reports adverse reactions to medications or treatments in accordance with the policy regarding the administration of medications by a licensed registered nurse.
Records patients medical information and vital signs.
RN Requirements:
Current RN license in the State.
Up-to-date first aid and CPR certification.
At least one year of experience as an RN in a long-term care setting is preferred.
Knowledge of Matrixcare preferred.
