Job description

Opportunities for Inclusion is seeking a compassionate, collaborative, and detail-oriented Registered Nurse (RN) to oversee healthcare management for adults with intellectual and developmental disabilities (I/DD) across our Department of Developmental Services (DDS)- funded Residential Services and Day Habilitation Programs.

In this dual-impact role, you will serve as the primary clinical authority, providing direct medical oversight, case management, staff training (including MAP supervision), and clinical consultation. You will work closely with interdisciplinary teams, residential house managers, families, primary care providers, and state agencies to promote optimal health, independence, and person-centered care for the individuals we support.

Key Responsibilities
  • Clinical Care & Case Management:
  • Perform routine health assessments, develop nursing care plans, and monitor the health of assigned individuals across Day Habilitation and Residential settings.
  • Manage acute and chronic health conditions, medical orders, specialist referrals, and follow-up care.
  • Coordinate hospital admissions and discharge planning to ensure seamless transitions and medication reconciliation.
  • Regulatory & Compliance Oversight:
  • Ensure full compliance with MassHealth, DDS, DPH, and agency healthcare standards.
  • Conduct two-year Service Needs Assessments (SNA) and contribute to Day Habilitation Service Plans (DHSP) and Individual Support Plans (ISP).
  • Maintain comprehensive, accurate electronic health records (EHR) and clinical documentation.
  • Medication Administration Program (MAP) & Staff Training:
  • Oversee MAP compliance across residential group homes; perform regular audits of Medication Administration Records (MARs) and medication storage.
  • Serve as a certified MAP Trainer (or obtain certification upon hire) to train, observe, and recertify direct care personnel.
  • Provide training to staff on specialized medical protocols (e.g., g-tube feeding, diabetes management, seizure response, dysphagia guidelines, and infection control).
  • Act as a clinical liaison between participants, guardians, direct support professionals, DDS service coordinators, and healthcare providers.
  • Participate in weekly reviews with residential house managers to audit medical appointments and medication changes.
Qualifications & Skills
  • Education & Licensure: Active, unencumbered Registered Nurse (RN) license in the Commonwealth of Massachusetts.
  • Experience:
  • Minimum of 1–2 years of nursing experience (preferred).
  • Prior experience working with individuals with intellectual and developmental disabilities (I/DD) or in community-based residential/day programs is strongly preferred.
  • Certifications: Current CPR/First Aid certification required.
  • DDS MAP certification or MAP Trainer endorsement highly desirable (training provided for qualified candidates).
  • Skills: Strong clinical assessment capabilities, knowledge of state regulations, excellent organizational skills, and a strong commitment to person-centered care.
  • Requirements: Valid Driver's License and reliable transportation (travel between local Waltham-area sites required). Must pass required background checks (CORI/SORI).
