Program Nurse (RN)

Shore Educational Collaborative is seeking a dedicated and compassionate Registered Nurse (RN) to join our Adult Services team at our Woburn-based Day Program. This role involves supporting adults with intellectual and developmental disabilities (I/DD) in a person‑centred, community‑oriented setting. The RN plays a key role in promoting the health and safety of program participants by providing direct nursing care, ongoing health monitoring, medication administration, and contributing to interdisciplinary team planning. The RN ensures compliance with state regulations, including those of MassHealth and the Massachusetts Department of Developmental Services (DDS).

Key Responsibilities
  • Direct Nursing Care: Administer prescribed medications and treatments, including G‑tube feedings and other medical interventions.
  • Provide first aid and emergency nursing care to participants and staff as needed.
  • Monitor chronic health conditions and support the management of acute medical issues.
  • Observe and document changes in health status, responding promptly and appropriately.
  • Complete initial and ongoing nursing assessments and prior authorizations for individuals in the Day Habilitation program.
  • Develop and update individualized healthcare plans in collaboration with the interdisciplinary team.
  • Maintain accurate and timely medical records in compliance with DDS, MassHealth, and Shore policies.
  • Write daily/monthly nursing progress notes and complete incident and injury reports as required.
  • Participate as a member of the Interdisciplinary Team to support person‑centred service planning and implementation.
  • Communicate regularly with families, guardians, residential providers, physicians, and other service providers to ensure coordinated care.
  • Provide consultation and guidance to staff on health‑related matters.
  • Provide health‑related training and in‑services for staff, including topics such as signs & symptoms, emergency response, seizure management, and universal precautions.
  • Facilitate health and wellness groups for individuals, such as nutrition education, hygiene, and safety skills.
  • Ensure that all health‑related activities and documentation meet state and agency regulatory requirements.
  • Monitor and promote adherence to safety protocols, infection control practices, and other health policies.
  • Participate in crisis response, behavioral interventions, and physical supports as related to necessary nursing support.
  • Assist individuals with activities of daily living (ADLs) and transfers when required.
  • Perform any other duties assigned by the Program Director or Health Care Coordinator.
Qualifications
  • Current Massachusetts RN License (Required)
  • Experience working with adults with I/DD (Preferred)
  • Knowledge of DDS and Day Habilitation regulations (Preferred)
  • Excellent written and verbal communication skills
  • Strong organizational, clinical, and interpersonal skills
  • Ability to work collaboratively within a multidisciplinary team
  • Must meet physical demands of the role, including lifting, transferring, and assisting with ADLs
  • CPR/First Aid certification (or willingness to obtain)
Work Schedule
  • Monday–Friday, typically 8:00 AM – 4:00 PM (no evenings, weekends, or holidays)

All adult program staff who may be expected to manage behavior must obtain and maintain certification in Shore's approved behavioral safety training program (Safety Care or equivalent). These skills are essential to ensuring the safety of all program participants and staff, and to meet state and federal requirements related to the safe implementation of behavior intervention plans.
