Job summary

Alternate Solutions Health Network is seeking a registered nurse to provide one-on-one home health care across Michigan communities including Birmingham, Franklin, Southfield, and surrounding areas. The role emphasizes coordinating care, leading in nursing activities, and delivering patient-centered services in the home environment.

You will work with an interdisciplinary team, receiving a guaranteed base salary plus an uncapped bonus structure. A $10,000 sign-on bonus is offered.

Qualifications

• Registered Nurse with current license in the state of employment.
• Minimum one-year experience as an RN in an acute care setting; home care experience preferred.
• Valid driver's license and auto insurance in your name as a driver.

Responsibilities

• Provide skilled nursing care in a patient's home as directed by the attending physician.
• Work autonomously, managing time efficiently.
• Perform OASIS assessments to develop and adjust care plans.
• Administer medications as prescribed and reduce hospital readmissions.
• Educate patient and family using teach-back methods.
• Document all clinical activities following agency protocol.
• Collaborate with interdisciplinary team to ensure optimal care.

You will work autonomously, so you need to be efficient in managing your time. You'll be responsible for all practices and duties within the scope of practice outlined by the state.MAJOR AREAS OF RESPONSIBILITY: Plan of Care: Observe and monitor patient conditions and perform OASIS assessments (If you don’t already know this assessment, we will teach you!) to develop an individualized care plan and adjust as needs change. Minimize Patient Risk: Administers medication as prescribed by the physician and helps decrease re-hospitalizations by prioritizing visits for high-risk patients. Continuity of Care: Manage multi-disciplinary care as applicable while promoting continuity of care with appropriate admissions, transfers, and discharges. Family Educator/Advocate: Educate the patient and family on the disease processes using teach back methods to ensure patient and family understanding. Policies: Completes all clinical documentation following agency protocol and Medicare/Federal guidelines. Collaborates with the interdisciplinary team and physicians to ensure optimal care is provided. Rules and Regulations: understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.
