Job summary

Alternate Solutions Health Network is seeking a licensed practical nurse (LPN) to join our home health team serving the Troy area. You’ll provide skilled nursing care in patients’ homes as directed by the attending physician, working autonomously with support from an interdisciplinary team.

Qualifications

• LPN with current state license.
• Minimum one-year experience as an LPN in acute care; home health preferred.
• Valid driver’s license and auto insurance in your name.
• Capable of all physical demands.

Responsibilities

• Provide skilled nursing care in patients' homes as directed by the attending physician.
• Educate patients and families on disease processes and preventive care; use teach-back methods.
• Complete clinical documentation following agency protocols and Medicare guidelines.
• Coordinate with case manager and schedulers to ensure timely patient admissions and care.

You will work autonomously, so you need to be efficient in managing your time. You'll be responsible for all practices and duties within the scope of practice outlined by the state.MAJOR AREAS OF RESPONSIBILITY:Plan of Care: Provide skilled nursing care as ordered by the attending physician and observe and monitor patient conditions and notify case manager of any changes in the patient’s condition.Minimize Patient Risk: Administer medication as prescribed by the physician and help decrease re-hospitalizations by educating the patient on preventative measures and making good decisions.Family Educator/Advocate: Educate the patient and family on the disease process using teach back methods to ensure patient and family understanding.Policies: Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.Operations: Attend in-service training and mandatory agency meetings.HARD & SOFT SKILLS:- Compassionate communicator with a positive attitude- Patience is a virtue when working with patients, families, physicians, and coworkers- Attention to detail is critical, as is being observant and following directionsREQUIREMENTS:1.
