Alternate Solutions Health Network is seeking an RN Case Manager to deliver skilled nursing care in patients’ homes across Macomb and Oakland Counties. You’ll lead care plans, coordinate with physicians and an interdisciplinary team, and participate in a patient-centered recovery journey with a focus on quality outcomes.

You’ll have on-call responsibilities and a guaranteed base salary plus an uncapped bonus.

Qualifications

• Registered Nurse with current license in the state of employment.
• Minimum one-year experience as an RN in an acute care setting; Home Care experience preferred.
• Valid driver's license and auto insurance in your name as a driver.
• Capable of all physical demands.

Responsibilities

• Plan of Care: Observe and monitor patient conditions, develop and adjust the care plan.
• Minimize Patient Risk: Administer meds as prescribed and prioritize visits for high-risk patients.
• Continuity of Care: Coordinate multi-disciplinary care and manage admissions, transfers, discharges.
• Family Educator/Advocate: Educate patient and family on disease processes and Care Plan.
• Supervision: Oversee total care by LPNs and aides; provide clinical leadership.
• Policies/Regulations: Follow agency policies and Medicare guidelines.

You will work autonomously, so you need to be efficient in managing your time. You will be responsible for all practices and duties within the scope of practice outlined by the state.MAJOR AREAS OF RESPONSIBILITY:Plan of Care: Observe and monitor patient conditions and perform OASIS assessments. develop an individualized care plan and adjust as needs change. Assume accountability and leadership for patient assessment and Plan of Care. Case manage and provide clinical care under physician direction.Minimize Patient Risk: Administers medication as prescribed by the physician and help decrease rehospitalization by prioritizing visits for high-risk patients. Update physicians’ orders in the patient chart. Prioritize visits to help decrease rehospitalization. Monitor Patient condition and safety. For hospice, evaluate whether the level of care continues to meet patient needs.Continuity of Care: Manage multi-disciplinary care as applicable while promoting continuity of care with appropriate admissions, transfers, and discharges. Collaborate with physicians and the interdisciplinary team. Coordinate care throughout the patient’s episode. Promote positive patient outcomes and continuity of services.Family Educator/Advocate: Educate the patient and family on the disease processes using teach back methods to ensure patient and family understanding. Incorporate patients and family into development of the Plan of Care. Educate families while supporting patient needs and preferences. Hospice: provide compassionate end of life care focused on the patient’s needs and preferred environment.Supervision: Oversee and supervise total care provided by LPNs and Nurse Aides. Provide clinical leadership for care delivered under the Plan of Care.Policies: Completes all clinical documentation following agency protocol and Medicare/Federal guidelines. Collaborates with the interdisciplinary team and physicians to ensure optimal care is provided. Follow agency policies and procedures.Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors. Follow applicable state requirements and federal/Medicare requirements.Operations: Attend in-service training and mandatory agency meetings. Work autonomously and efficiently manages time. Provide one-on-one home-based clinical care.
