Job summary

Heritage Hills Rehabilitation & Healthcare Center is seeking a registered nurse (RN) for full-time and part-time opportunities. Shifts available 3-11 and 11-7, with daily pay and shift differentials.

A $6,000 sign-on bonus is offered for full-time staff. The RN delivers skilled clinical care, monitors resident conditions, and follows individualized care plans, ensuring high-quality care in a 100-bed sub-acute and long-term facility in Rhode Island.

Qualifications

• RI RN license required.
• BLS CPR certification required.
• Ability to manage care for multiple residents in a fast-paced setting.
• Strong clinical assessment, communication and documentation skills.

Responsibilities

• Provide resident care in accordance with standards and physician plan.
• Follow and implement care plans from Nurse Manager.
• Administer medications and treatments per plan of care.
• Monitor and report changes in residents’ conditions to the clinical team.
• Maintain accurate and timely documentation per orders and policies.
• Collaborate with interdisciplinary teams for high-quality resident care.
• Ensure compliance with infection control and safety regulations.

Guided by our core values of Passion, Respect, and Excellence, the Registered Nurse (RN) is responsible for delivering skilled clinical care, monitoring resident conditions, and ensuring all care is provided in accordance with established standards and individualized care plans.

Responsibilities for Registered Nurse (RN):

• Provide resident care in accordance with acceptable standards of nursing practice and the physician’s plan of treatment, as well as the written plan of care.

• Follow and implement care plans developed by the Nurse Manager.

• Perform medication administration and treatments in accordance with the plan of care (POC).

• Monitor and assess residents’ conditions, promptly reporting changes to appropriate clinical team members.

• Maintain accurate, timely, and complete documentation in accordance with physician orders, facility policies, and regulatory standards.

• Collaborate with interdisciplinary teams to ensure coordinated and high-quality resident care.

• Ensure compliance with infection control protocols, safety standards, and nursing regulations.

Qualifications for Registered Nurse (RN):

• A current, unencumbered active license to practice as an RN in the state of Rhode Island.

• Valid BLS Certification (CPR) required.

• Ability to manage care for multiple residents simultaneously in a fast-paced healthcare environment.

• Strong clinical assessment, communication, and documentation skills.
