Job summary

Southampton Rehabilitation and Healthcare Center is seeking an experienced Registered Nurse (RN) to deliver skilled clinical care to residents in a Sub-Acute and Long-Term Care setting. The RN will monitor resident conditions, implement individualized care plans, and ensure documentation and compliance with standards.

Responsibilities include medication administration, collaboration with the care team, and adherence to infection control and safety protocols.

Qualifications

• Active RN license in Virginia (unencumbered).
• Valid BLS CPR certification.
• Ability to manage care for multiple residents in a fast-paced setting.
• Strong clinical assessment and documentation skills.

Responsibilities

• Follow care plans developed by the Nurse Manager.

Responsibilities for Registered Nurse (RN):

• Provide resident care in accordance with acceptable standards of nursing practice and the physician’s plan of treatment, as well as the written plan of care.
• Follow and implement care plans developed by the Nurse Manager.
• Perform medication administration and treatments in accordance with the plan of care (POC).
• Monitor and assess residents’ conditions, promptly reporting changes to appropriate clinical team members.
• Maintain accurate, timely, and complete documentation in accordance with physician orders, facility policies, and regulatory standards.
• Collaborate with interdisciplinary teams to ensure coordinated and high-quality resident care.
• Ensure compliance with infection control protocols, safety standards, and nursing regulations.

Qualifications for Registered Nurse (RN):

• A current, unencumbered active license to practice as an RN in the state of va.
• Valid BLS Certification (CPR) required.
• Ability to manage care for multiple residents simultaneously in a fast-paced healthcare environment.
• Strong clinical assessment, communication, and documentation skills.
