Job summary

PurposeCare is seeking a Home Health Registered Nurse to deliver skilled visits in patients' homes across Indiana. The role requires an active state RN license, at least one year of nursing experience, and the ability to manage physical tasks and transportation to patient locations.

This per-visit position offers flexible scheduling, autonomy in clinical decisions, and opportunities for professional growth within PurposeCare's home health team.

Qualifications

• RN license in the state
• 1 year of nursing experience required
• Home health experience and OASIS experience preferred
• Ability to perform physical tasks including lifting up to 50 lbs
• Valid driver's license and transportation access
• Background check clearance required

Responsibilities

• Provide skilled visits addressing the full scope of patient needs in the home
• Build meaningful, one-on-one connections with patients in their own homes
• Deliver holistic care and patient-centered services

Qualifications and Requirements:

• Current license with the state Board of Nursing
• 1 year of nursing experience is required - home health and OASIS experience preferred
• Ability to perform tasks involving physical activity, which may include heavy lifting up to 50 lbs, bending, and standing
• Current Driver's License, vehicle insurance, and/or access to dependable transportation
• Ability to successfully complete a background check

Hiring Range

This is a paid per visit position - Rates range from $50 - $95 base
