LionStone Care in Paulding, OH is seeking a full-time Registered Nurse to deliver patient-centered nursing care in a facility setting. Responsibilities include assessments, planning, medication administration, and collaboration with interdisciplinary teams.

Qualifications

• Graduate of an accredited school of nursing (ADN or BSN).
• Active RN license and current, unrestricted.

Responsibilities

• Assess patient health status and develop care plans with the interdisciplinary team.
• Administer medications and therapies per orders and policies.
• Monitor responses and adjust care as needed; document in electronic health records.
• Perform wound care, IV therapy, catheter care and respiratory treatments per protocols.
• Provide patient education on medications and treatments to promote health.
• Collaborate with care teams and participate in care conferences to discuss progress.
• Respond to emergencies with CPR and stabilization as needed.
• Advocate for patients, ensuring respectful, person-centered care.

Key Responsibilities

Patient Care

• Conduct thorough assessments of patient or resident health status, including physical, mental, and emotional health.
• Develop individualized care plans based on patient needs, collaborating with the interdisciplinary team to ensure comprehensive care.
• Administer medications, treatments, and therapies according to physician orders, facility policies, and regulatory guidelines.
• Monitor patient responses to treatments and adjust care plans as needed, reporting any changes in condition to the appropriate healthcare team members.

Clinical Procedures

• Perform clinical procedures such as wound care, IV therapy, catheter care, and respiratory treatments in accordance with facility protocols and best practices.
• Provide patient and family education on medications, treatments, and lifestyle adjustments to promote health and wellness.
• Assist with diagnostic testing, monitor vital signs, and ensure proper documentation of patient care activities.

Medication Administration

• Safely administer medications and treatments, following the five rights of medication administration (right patient, right drug, right dose, right route, and right time).
• Ensure accurate documentation of medication administration and promptly report any adverse reactions or errors.
• Monitor and manage medications for patients with chronic conditions or complex medication regimens.

Documentation and Compliance

• Maintain accurate, thorough, and timely documentation of all patient care activities in the electronic health record (EHR) or other required formats.
• Ensure compliance with federal, state, and local regulations, as well as facility policies related to patient care and documentation.
• Participate in quality assurance initiatives to maintain and improve the standard of care.

Collaboration and Communication

• Communicate effectively with patients, families, and the interdisciplinary care team to ensure a coordinated and holistic approach to care.
• Act as an advocate for patients and their families, ensuring their needs and preferences are met.
• Participate in care conferences and team meetings to discuss patient progress, care plans, and any necessary adjustments.

Emergency Response

• Respond promptly to medical emergencies, providing life‑saving interventions such as CPR, airway management, and advanced resuscitation measures as necessary.
• Assist in stabilizing and transferring patients to higher levels of care when required.

Patient Advocacy

• Advocate for the rights and needs of patients, ensuring they receive respectful, compassionate, and person‑centered care.
• Provide emotional support to patients and families, helping them navigate difficult health situations and decisions.

Qualifications

• Education: Graduate of an accredited school of nursing (Associate’s or Bachelor’s degree in Nursing).
• Licensure: Active and unrestricted Registered Nurse (RN) license.
• Experience: Previous experience in a healthcare, long‑term care, or acute care setting is preferred but not required. New graduates are welcome to apply.
