Job summary

LionStone Care is seeking a Registered Nurse (RN) to provide high-quality, patient-centered care in a healthcare or long-term care setting. The RN assesses needs, develops care plans, administers treatments, and collaborates with a multidisciplinary team to promote patient health.

Responsibilities include patient assessment, med administration, wound care, patient education, documentation, and emergency response. Active RN license required; new grads encouraged.

Qualifications

  • Graduate of an accredited nursing program (Associate's or Bachelor’s degree in Nursing).
  • Active Registered Nurse (RN) license.
  • Experience in healthcare or long-term care preferred, but new graduates are encouraged to apply.
  • Strong clinical nursing skills and ability to collaborate with a multidisciplinary team.

Overview

The Registered Nurse (RN) provides high-quality, patient-centered care in a healthcare or long-term care setting. The RN is responsible for assessing patient needs, developing individualized care plans, administering treatments, and coordinating with other healthcare professionals to deliver comprehensive care. This role requires strong clinical skills, compassion, and the ability to collaborate with a multidisciplinary team to promote patient health and well-being.

Key Responsibilities
  • Patient Assessment and Care Planning
    • Conduct initial and ongoing assessments of patients\' physical, emotional, and psychological health.
    • Develop, implement, and evaluate individualized care plans based on patient needs, medical conditions, and treatment goals.
    • Monitor patient progress and modify care plans as necessary in collaboration with the healthcare team.
  • Administration of Treatments and Medications
    • Administer medications, treatments, and therapies as prescribed by physicians, ensuring proper dosages and monitoring for side effects.
    • Perform medical procedures such as wound care, injections, catheterization, and IV therapy.
    • Maintain accurate records of medications administered and treatments provided, ensuring compliance with facility and regulatory standards.
  • Patient Education and Support
    • Educate patients and their families on diagnoses, treatments, medications, and self-care strategies.
    • Provide emotional support and guidance to patients and families, helping them understand treatment plans and manage expectations.
    • Promote health and wellness by encouraging lifestyle changes and preventive care measures.
  • Collaboration and Communication
    • Work closely with physicians, nurse practitioners, nursing assistants, and other healthcare professionals to coordinate care and ensure a cohesive approach to treatment.
    • Communicate effectively with patients, families, and the healthcare team regarding patient status, care plans, and any changes in condition.
    • Participate in interdisciplinary team meetings and patient care conferences, contributing nursing expertise to the decision-making process.
  • Documentation and Compliance
    • Maintain accurate, detailed, and timely documentation of patient care, including assessments, treatments, medications, and patient progress.
    • Ensure compliance with local, state, and federal regulations, as well as facility policies and procedures, regarding patient care and documentation.
    • Participate in audits, inspections, and surveys as required to ensure the facility meets regulatory standards.
  • Emergency Response and Critical Care
    • Respond quickly and appropriately to medical emergencies, providing life-saving interventions such as CPR or advanced first aid.
    • Monitor and assess patients in critical condition, communicating any changes immediately to the healthcare team and adjusting care as needed.
    • Serve as a resource for nursing staff and assist with complex clinical situations as required.
Qualifications
  • Education: Graduate of an accredited nursing program (Associate\'s or Bachelor’s degree in Nursing).
  • Licensure: Active Registered Nurse (RN) license.
  • Experience: Previous experience in a healthcare or long-term care setting is preferred, but new graduates are encouraged to apply.
  • Skills:
