Job summary

Xclusive Care Services is seeking part-time nursing professionals to support medically fragile children in Georgia, traveling to patient homes. The role encompasses direct patient care, medication administration, and coordination with the care team.

Applicants should have a Georgia RN license or LPN, CPR certification, and 1–2 years of related experience. Reliable transportation and a valid driver’s license are required. This is a part-time, contract position with on-the-road work.

Qualifications

• Education: Bachelor’s degree in nursing (Preferred) or LPN.
• Experience: 1–2 years of related experience; or an equivalent combination of education and experience.
• CPR Certification (Required).
• Excellent communication skills and professional demeanor.

Responsibilities

• Provide direct nursing care, including administering medications, managing ventilators, feeding tubes, tracheostomies, and other medical devices.
• Conduct Supervisory Visits every two weeks (plus monthly, or every 60 days) to our patients at their homes.
• Monitor and assess the child’s condition regularly, recognizing and responding to any changes in status.
• Perform and document clinical assessments, including vital signs, pain management, and overall well-being.
• Implement and follow the child's care plan as prescribed by the physician.
• Coordination of Care: Collaborate with the healthcare team and other nurses, to ensure comprehensive care.
• Communicate any changes in the child’s condition to the physician and healthcare team promptly.
• Maintain accurate and thorough documentation of the child’s care, including progress notes, medication records, and other required documentation.

Patient Care

• Modifies patient treatment plans as indicated by patients' responses and conditions.
• Monitors all aspects of patient care.
• Prepares patients for and assists with examinations or treatments.

Job Qualifications
