Job summary

Exton Post Acute is seeking a Registered Nurse (RN) to deliver skilled nursing care in a compassionate, resident-centered environment. The role supports resident health, safety, and wellbeing, guided by core values of Passion, Respect, and Excellence.

The RN will administer medications, monitor conditions, and ensure care plans are followed, collaborating with an interdisciplinary team in a 120-bed sub-acute/long-term care setting. Competitive pay and benefits are provided.

Qualifications

• Must hold an active PA RN license.
• BLS/CPR certification required.
• Ability to manage multiple residents in a fast-paced setting.
• Strong documentation and communication skills.

Responsibilities

• Provide resident care per physician orders and written plans of care.
• Follow and implement care plans developed by the Nurse Manager.
• Administer medications and treatments per plan of care.
• Monitor residents’ conditions and promptly report changes to the clinical team.
• Maintain accurate, timely documentation per orders, policies, and regulations.
• Collaborate with interdisciplinary teams to ensure coordinated, high-quality care.

Responsibilities for Registered Nurse (RN):

• Provide resident care in accordance with acceptable standards of nursing practice and the physician’s plan of treatment, as well as the written plan of care.
• Follow and implement care plans developed by the Nurse Manager.
• Perform medication administration and treatments in accordance with the plan of care (POC).
• Monitor and assess residents’ conditions, promptly reporting changes to appropriate clinical team members.
• Maintain accurate, timely, and complete documentation in accordance with physician orders, facility policies, and regulatory standards.
• Collaborate with interdisciplinary teams to ensure coordinated and high-quality resident care.
• Ensure compliance with infection control protocols, safety standards, and nursing regulations.

Qualifications for Registered Nurse (RN):

• A current, unencumbered active license to practice as an RN in the state of PA.
• Valid BLS Certification (CPR) required.
• Ability to manage care for multiple residents simultaneously in a fast-paced healthcare environment.
• Strong clinical assessment, communication, and documentation skills.
