The Registered Nurse (RN) in Home Health provides and directs nursing care to patients in their homes, as prescribed by physicians and in compliance with laws, regulations, and agency policies.

Responsibilities include:

• Coordinating the total care plan with healthcare professionals and ensuring continuity of care through effective communication.
• Conducting initial and comprehensive nursing evaluations, developing care plans, and submitting accurate documentation.
• Performing patient assessments and developing appropriate care plans.
• Ensuring patients meet home health eligibility and medical necessity guidelines.
