Job summary

Village Caregiving in Wisconsin seeks a compassionate RN to join our growing team for in-home patient care. You will visit patients in their homes, perform initial and ongoing assessments, and support treatment plans with vital signs and medication reviews.

The ideal candidate has an RN license, CPR certification, and excellent communication skills, with a valid driver’s license and ability to travel locally. A flexible schedule is offered along with referral programs and daily pay options.

Qualifications

• Graduate of an accredited Diploma, Associate, or Bachelor’s School of Nursing, or vocational nursing.
• Current state license as an RN
• Proof of CPR certification
• Excellent communication skills
• Current driver’s license and reliable transportation

Responsibilities

• Visit patients in their homes for one-on-one visits
• Complete initial and ongoing assessments to identify needs of home health patients
• Assess patients’ condition, chart observations, and perform evaluation tasks including vital signs and medication review
• Educate patients and families on home health care strategies and procedures and recommend devices or tools when appropriate
• Provide effective communication to patients, families, team members, and other health care professionals

• Provide effective communication to patients/clients, their family members, team members, and other health care professionals
