Job summary

Village Caregiving, serving the Menomonie area, is seeking a compassionate RN to join our growing home health team. The successful candidate will visit patients in their homes, perform assessments, and collaborate with families and health professionals to deliver high-quality care.

Requirements include an RN license, CPR certification, valid driver's license, and strong interpersonal skills. We offer a flexible schedule, referral program, and daily pay options to support our caregivers.

Qualifications

• Graduate of an accredited Nursing program (Diploma/Associate/Bachelor) or vocational nursing.
• Current state RN license.
• Proof of CPR certification.
• Compassionate, friendly demeanor.
• Excellent communication skills.
• Valid driver’s license and reliable transportation.
• Ability to travel locally and maintain a flexible schedule.

Responsibilities

• Visit patients in their homes for one-on-one visits.
• Complete initial and ongoing assessments to identify the physical, psychosocial, and environmental needs of home health patients.
• Assess patients’ conditions and chart observations, and perform evaluation tasks including vital signs and medication review.
• Educate patients and their families on proper home health care strategies and procedures, and make recommendations for devices or tools that might improve the patient’s quality of life.
• Provide effective communication to patients/families, team members, and other health care professionals.
