Job summary

Hireology is seeking a compassionate RN to join our growing in-home care team in Wisconsin. The ideal candidate will visit patients in their homes for one-on-one assessments and provide skilled nursing care with a friendly, can-do attitude.

You will perform initial and ongoing assessments, monitor vitals, review medications, educate patients and families, and communicate effectively with the care team to improve quality of life. A valid RN license and reliable transportation are required.

Qualifications

• Graduate of an accredited nursing program with RN eligibility.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Ability to work a flexible schedule and travel locally.
• Excellent communication and interpersonal skills.

Responsibilities

• Visit patients in their homes for one-on-one visits
• Complete initial and ongoing assessments to identify the physical, psychosocial, and environmental needs of home health patients
• Assess patients’ conditions and chart observations, and perform evaluation tasks including vital signs and medication review
• Educate patients and their families on proper home health care strategies and procedures, and make recommendations for devices or tools that might improve the patient’s quality of life
• Provide effective communication to patients/clients, their family members, team members, and other health care professionals
