Schedule Details:
This is a pool position requiring a minimum of 6 shifts per month; additional shifts may be needed for staff illness or PTO coverage. Shifts are Monday-Friday, 8-4:30. The employee will be included in weekend and holiday rotations. This position is NOT fully remote; the candidate must work on-site at Waukesha Memorial Hospital.
What You Will Do:
This role requires on-site work in Waukesha, WI. The Remote Patient Monitoring (RPM) RN will assess patient data via digital technology to manage care for acute and chronically ill patients. Responsibilities include patient assessment, education, medication management, goal setting, and care planning. The RN will collaborate with physicians and care teams, supporting self-management of medical conditions using motivational techniques. They will interpret submitted vital signs and symptoms, respond to alerts, and escalate care as needed. Early detection of adverse health trends aims to reduce hospital visits and readmissions. The RN will build long-term relationships with patients through regular communication.
