Schedule Details:

This is a pool position requiring a minimum of 6 shifts per month, with potential for increased shifts due to staff illness or PTO coverage. Shifts are Monday-Friday, 8-4:30. The employee will be included in department weekend and holiday rotations. This position is NOT fully remote; the candidate must work on-site at Waukesha Memorial Hospital.

What You Will Do:

This role requires on-site work at Waukesha, WI. The Remote Patient Monitoring (RPM) RN will assess patient data provided via digital technology to manage care for acute and chronically ill patients. Responsibilities include patient assessment, education, medication management, goal setting, and care planning. The RN collaborates with physicians and care team members to support self-management of medical conditions, using motivational communication and positive reinforcement.

The RPM RN will interpret vital signs and symptoms submitted by patients, respond to alerts, and escalate care as needed. Early detection of adverse health trends aims to reduce hospital visits and readmissions. The RN will build long-term relationships with patients through regular communication, becoming trusted members of the care team.

Additional Requirements:
  • Graduate from an accredited nursing school; Bachelor’s degree preferred.
  • 1-2 years’ experience in hospital or ambulatory care, working with patients with chronic diseases.
  • Experience in remote patient monitoring and electronic medical records; Epic experience preferred.
  • Strong technological aptitude and ability to adapt to new software and tools.
