Job summary

Home & Care Home Health in Casper, WY is seeking a full-time Registered Nurse (RN) to provide holistic in-home care to seniors. You will assess patient needs, develop care plans, administer medications, and coordinate with families and the care team to ensure high-quality, personalized service.

Job Summary

Home & Care Home Health is seeking a full time compassionate and skilled Registered Nurse (RN) to join our home health team in Casper, WY. You will provide high-quality patient care to seniors in their homes, allowing them to receive the care they need in the comfort of their own surroundings. Responsibilities

• Provide skilled nursing care to seniors in their homes, assessing their physical, emotional, and social needs


• Develop and implement individualized care plans in collaboration with patients, families, and interdisciplinary team members


• Administer medications, treatments, and therapies as prescribed by physicians, and monitor patient responses


• Educate patients and families on disease management, self-care, and wellness strategies


• Collaborate with other healthcare professionals to ensure comprehensive care and coordination of services


• Maintain accurate and detailed records of patient interactions, assessments, and care plans

Requirements

• Current RN license in the state of Wyoming


• Minimum of 0 years of experience in home care or a related setting (new graduates welcome to apply!)


• Ability to work flexible hours, including evenings, weekends, and holidays


• Valid driver's license and reliable transportation (mileage reimbursed)
