Home&Care is seeking a compassionate Registered Nurse (RN) to provide in-home patient care to Casper residents. You will assess, plan, implement, and evaluate care, delivering treatment with safety and comfort while coordinating with families and the clinical team.

Qualifications

• Current RN license in Wyoming state.

Responsibilities

• Provide direct patient care, including assessment, planning, implementation, and evaluation of care for residents in home settings.
• Develop and implement individualized care plans with the interdisciplinary team.
• Administer medications, treatments, and therapies as prescribed by physicians.
• Monitor and document patient progress, reporting changes in condition.
• Communicate effectively with patients, families, and healthcare team members.
• Participate in quality improvement initiatives to enhance patient care and satisfaction.
• Collaborate with other departments to ensure seamless transitions and optimal care.
