Job summary

Brooks Home Health in Orange Park, FL seeks a dedicated Registered Nurse to directly manage patient care in collaboration with a multidisciplinary team. The role functions within the Florida Nurse Practice Act and established care plans from admission through discharge.

You will identify nursing and clinical problems, develop individualized plans, and document progress while maintaining professional communication with patients, families, and care teams.

Qualifications

• Registered Nurse with Florida license.
• Basic Life Support (BLS) certification required.
• Nursing degree from an accredited school.

Responsibilities

• Plan and manage nursing care following the Florida Nurse Practice Act and chain of command.
• Identify nursing/clinical problems from assessments and document findings.
• Establish comprehensive individualized plan of care within 24 hours of admission.
• Develop patient-centered goals with clients and families to promote independence.
• Complete medication reconciliation at each SN visit and report changes to the team.
• Administer medications and treatments as ordered and monitor patient response.
• Collaborate with interdisciplinary team in care planning and delivery.
• Teach/Discharge planning including complex needs of patient and family.
• Document progress and outcomes according to guidelines.
