Job summary

ChristianaCare, Hospital Care at Home Program, seeks a part-time Registered Nurse (RN) to deliver hospital-level care to medical-surgical patients in the home setting from Newark, Delaware. The role operates in a hybrid model with day shifts and weekend/holiday coverage per department guidelines.

You will join a multidisciplinary team and provide in-home visits, telehealth monitoring, and coordination with physicians and allied staff to ensure safe, high-quality care in the patient's home.

Qualifications

• RN license in Delaware or a compact state.
• BSN preferred or committed to acquiring BSN within 3 years.
• BLS certification is required.
• Active driver’s license is required.

Responsibilities

• Provide care to medical-surgical patients in patients' homes.
• Deliver hospital-level care same as in-hospital, at home.
• Collaborate with a care team including physicians and NPs.
• Coordinate visits and telehealth monitoring per program guidelines.

Education

RN License in Delaware or Compact State

PT Day (Work Hours: two 20-hour shifts per week), Weekends & Holidays per department guidelinesLocation: Hybrid – HCC in Newark DE/Visiting patients’ homesChristianaCare - Hospital Care at Home Program is looking for a PT Registered Nurse (RN) to provide care to medical-surgical patients in the comfort of their homes. Our Hospital Care at Home Program enables patients to receive hospital-level care from the comfort of their homes rather than in ahospital. Our team of experts will treat and monitor patient health with the highest level of care.

Our Hospital Care at Home services include:

• A care team comprises physicians, nurse practitioners, physician assistants, paramedics, and registered nurses.
• Laboratory, infusion therapy, wound care, imaging, PT, OT, and EKG services.
• Advanced practice provider in-home visit.
• Virtual specialty provider consults.
• Medication delivery services.

While patients may not be physically present in the hospital, they remain continuously connected to their healthcare providers. Hospitalized at Home patients can connect with...

• (1) In-person Home Visits to Patient Hospitalized at Home (2 per week)

Education & Experience:

• RN License in Delaware or a Compact State
• BSN preferredor committed to acquiring BSN within 3 years of the hire date.
• A minimum of three years of med/surg experience as an RN is required. Critical Care experience is a plus.
• BLS required.

Hourly Pay Range: $42.11 - $67.37

This pay rate/range represents ChristianaCare’s good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
