Job summary

ChristianaCare in Newark, DE is seeking a part-time Registered Nurse (RN) for the Hospital Care at Home Program. You will provide hospital-level care to medical-surgical patients in their homes, coordinating care with physicians, nurse practitioners, and other team members.

This hybrid role requires Delaware RN license (compact state ok), BLS, and a minimum of three years in med/surg. BSN preferred or pursued within three years.

Qualifications

• RN license in Delaware or a compact state.
• BSN preferred or commit to BSN within 3 years.
• Minimum of three years med/surg experience as an RN.
• BLS certification is required.
• Active driver's license.

Responsibilities

• Provide hospital-level care to medical-surgical patients in patients' homes.
• Attend two 20-hour shifts per week and participate in weekends/holidays per guidelines.
• Coordinate with physicians, nurse practitioners, PAs, nurses, and other team members.
• Utilize telehealth and in-home visits to monitor patient health.

PT Day (Work Hours: two 20-hour shifts per week), Weekends & Holidays per department guidelines

Location: Hybrid - HCC in Newark DE/Visiting patients' homes

ChristianaCare - Hospital Care at Home Program is looking for a PT Registered Nurse (RN) to provide care to medical-surgical patients in the comfort of their homes. Our Hospital Care at Home Program enables patients to receive hospital-level care from the comfort of their homes rather than in a hospital. Our team of experts will treat and monitor patient health with the highest level of care.

Our Hospital Care at Home services include:

• A care team comprises physicians, nurse practitioners, physician assistants, paramedics, and registered nurses.
• Laboratory, infusion therapy, wound care, imaging, PT, OT, and EKG services.
• Advanced practice provider in-home visit.
• Virtual specialty provider consults.
• Medication delivery services.

While patients may not be physically present in the hospital, they remain continuously connected to their healthcare providers. Hospitalized at Home patients can connect with...

• (1) In-person Home Visits to Patient Hospitalized at Home (2 per week)
• (2) Virtual Visits
• (3) Monitoring through our Telehealth Platform.

Education & Experience:

• RN License in Delaware or a Compact State
• BSN preferred or committed to acquiring BSN within 3 years of the hire date.
• A minimum of three years of med/surg experience as an RN is required. Critical Care experience is a plus.
• BLS required.

Hourly Pay Range: $42.11 - $67.37

This pay rate/range represents ChristianaCare's good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
