Job summary

ChristianaCare’s Hospital Care at Home Program in Newark, DE is seeking a full-time Registered Nurse to deliver hospital-level care in patients’ homes and oversee in-home visits across a 40-hour workweek. The role blends in-person home visits with hospital-level oversight, following department guidelines for weekends and holidays.

• RN license in Delaware or a compact state.
• BSN preferred or committed to obtaining BSN within 3 years.
• Minimum of three years med/surg RN experience; CCU/critical care experience is a plus.
• BLS certification.
• Active driver's license.

Responsibilities

• Provide care to medical-surgical patients in the home setting.
• Deliver hospital-level care in the home as part of Hospital Care at Home Program.
• Coordinate with interdisciplinary team to monitor patient health.

Education

RN license in Delaware or compact state

Registered Nurse (RN) – Hospital Care at Home Program FT Day (Work Hours: 40-hour shifts per week), Weekends & Holidays per department guidelines

Location: Hybrid – HCC in Newark DE/Visiting patients’ homes

ChristianaCare – Hospital Care at Home Program is looking for a FT Registered Nurse (RN) to provide care to medical-surgical patients in the comfort of their homes. Our Hospital Care at Home Program enables patients to receive hospital-level care from the comfort of their homes rather than in a hospital. Our team of experts will treat and monitor patient health with the highest level of care.

Hospital Care at Home services include:

• A care team comprises physicians, nurse practitioners, physician assistants, paramedics, and registered nurses.
• Laboratory, infusion therapy, wound care, imaging, PT, OT, and EKG services.
• Advanced practice provider in-home visit.
• Virtual specialty provider consults.
• Medication delivery services.

Education & Experience:

• RN License in Delaware or a Compact State
• BSN preferred or committed to acquiring BSN within 3 years of the hire date.
• A minimum of three years of med/surg experience as an RN is required. Critical Care experience is a plus.
• BLS required.

Hourly Pay Range:

Hourly Pay Range: $42.11 - $67.37

This pay rate/range represents ChristianaCare’s good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
