Job summary

Mayo Clinic in Minnesota is seeking a skilled behavioral health professional to support Integrated Behavioral Health (IBH) care coordination, conducting comprehensive patient assessments and coordinating care to align with each patient’s needs.

You will collaborate with supervising psychiatrists to determine eligibility for IBH care coordination, perform mental health screenings, and ensure accurate medication reconciliation.

Responsibilities

• Care Coordination & Assessment
• Conduct comprehensive assessments for patients referred for IBH care coordination, including mental health screening tools (PHQ-9, GAD-7, C-SSRS, etc.) and medication reconciliation.
• Determine eligibility for IBH care coordination for bipolar disorder and other conditions, consulting with supervising psychiatrists as needed.
