Mayo Clinic in Minnesota is seeking a dedicated IBH Care Coordinator to support patient-centered care across care teams.

You will perform comprehensive assessments for referred patients, use validated mental health screening tools, and coordinate medications reconciliation. You will also determine eligibility for IBH care coordination and collaborate with supervising psychiatrists to optimize treatment plans.

Qualifications

• Care Coordination & Assessment: Conduct comprehensive assessments for patients referred for IBH care coordination, including mental health screening tools (PHQ-9, GAD-7, C-SSRS, etc.) and medication reconciliation.
• Collaborative Care Management: Determine eligibility for IBH care coordination for bipolar disorder and other conditions, consulting with supervising psychiatrists as needed.
