Job summary

Mayo Clinic in Minnesota is seeking a care coordination professional to conduct comprehensive assessments for patients referred for Integrated Behavioral Health (IBH) care coordination, using PHQ-9, GAD-7, and related tools to support medication reconciliation.

You will determine eligibility for IBH care coordination for bipolar disorder and other conditions, consulting with supervising psychiatrists as needed.

Responsibilities

• Conduct comprehensive assessments for patients referred for IBH care coordination, including mental health screening tools (PHQ-9, GAD-7, C-SSRS, etc.) and medication reconciliation.
• Determine eligibility for IBH care coordination for bipolar disorder and other conditions, consulting with supervising psychiatrists as needed.
• Oversee Collaborative Care Management activities among care teams.
