Mayo Clinic is seeking professionals to support IBH care coordination, performing comprehensive patient assessments, including mental health screening tools, and medication reconciliation.

You will determine eligibility for IBH care coordination for bipolar disorder and other conditions, consulting with supervising psychiatrists as needed, and participate in collaborative care management to enhance patient outcomes.
