Job summary

Elara Caring is seeking a dedicated Registered Nurse to provide patient care across the continuum in home health and hospice settings. You will work with patients in their homes, delivering compassionate, high-quality care and coordinating with the Clinical Supervisor to ensure the right care at the right time.

This role requires a valid RN license, at least one year of clinical experience, and willingness to travel up to 50%.

Qualifications

• Graduate of accredited nursing program.
• Current State License as a Registered Nurse (RN).
• Minimum 1 year of clinical care experience.
• Experience in hospice or home health preferred.

Responsibilities

• Provide patient care across the continuum in home health/hospice settings.
• Collaborate with the Clinical Supervisor to ensure timely, right-care delivery.
• Maintain high-quality patient care and safety standards in patients' homes.
• Remain flexible with travel up to 50% as required.

What is Required?

• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
• Current State License as a Registered Nurse RN
• 1 year of experience in a clinical care setting
• Experience in a hospice or home health environment is preferred
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Reliable transportation to perform job tasks
• 50% travel
• You will report to the Clinical Supervisor.
