Proforma Health Inc. seeks an experienced RN for the Resource Team/Float Pool on a per diem basis in Arlington Heights, IL. The role covers night shift hours from 7:00pm to 7:30am, providing flexible staffing support across units.

The ideal candidate holds an Associate's degree or BSN, holds an Illinois nursing license, and CPR certification. You will conduct assessments, coordinate care plans, and educate patients and families while ensuring compliance with nursing standards.

Qualifications

• Must complete an accredited nursing program and hold an Illinois nursing license.
• CPR certification is required.
• Currently licensed to practice in Illinois.

Responsibilities

• Conduct patient assessments and collect health data.
• Coordinate and tailor patient care plans for safe, timely interventions.
• Communicate with patients, families, and healthcare providers in person, over the phone, and in writing.
• Provide patient and family education based on assessed needs.
• Participate in quality improvement initiatives.
