Summary

The Phoenix VA Health Care System is currently recruiting for a full-time Registered Nurse-Patient Safety Manager position with the Qualify and Patient Safety Service, located at the Phoenix Division. The Patient Safety Manager provides systems thinking and analytical support to oversee all Patient Safety activities within the facility and associated care locations.

Qualifications

Basic Requirements:

• English Language Proficiency
• Graduate of an accredited school of professional nursing (ACEN or CCNE)
• Possession of a valid state license; candidates in a master's bridge program may be appointed temporarily
• Foreign nursing graduates must have current unrestricted registration and degree validation
• Current full, active, and unrestricted nursing license in the U.S.
• Graduate Nurse Technician (GNT) exception applies for candidates pursuing licensure
• Preferred Experience: 4-5 years of acute inpatient nursing, MSN or healthcare-related master's, experience with data collection, root cause analysis, healthcare failure modes
• Experience with report preparation, trend analysis, and patient safety follow-up

Grade Determinations: Detailed criteria available online for each grade level, including education, experience, and practice dimensions.

Physical Requirements: Visual acuity, hearing, speech, manual dexterity, occasional physical exertion, exposure to infectious materials, and ability to work under stress.

Duties

• Manage all aspects of the Patient Safety program, including standards, policies, and systems as directed by NCPS and The Joint Commission
• Investigate adverse events and near misses, ensuring adherence to safety goals
• Lead root cause analyses and implement systemic improvements
• Monitor and facilitate compliance with patient safety processes
• Lead safety improvement initiatives and update facility leadership
• Participate in the execution of the Patient Safety Program and perform additional duties as assigned
