Responsibilities

• Provide direct nursing care as part of the interdisciplinary team to a variety of patients.
• Collect comprehensive data relevant to the patient’s health.
• Assess health status and document clinical findings.
• Develop and coordinate a plan of care with patient and family goals.
• Communicate and coordinate with care team to ensure continuity of care.
• Document patient care and outcomes per protocol.
