You will collaborate with inter-professional teams, support discharge planning, and utilize health IT to monitor outcomes and drive quality improvements.

Qualifikationen

• Specialized Diploma Graduate of an accredited school of nursing required.
• Patient- and family-centered care knowledge to collaborate with patients and families in care planning.
• Cultural awareness for diverse backgrounds and respectful connection.
• Effective communication to transmit, receive, and interpret ideas with patients and families.
• Patient assessment and care knowledge to assess ailments and implement plans.
• Education of patients and care partners on conditions, treatments, and next steps.
• Knowledge of medications and prescription considerations for discharge.
• Discharge planning to coordinate post-discharge needs and follow-up.
• Knowledge of assessment tools and clinical judgement in care planning.
• Health information documentation methods for medical histories and treatments.
• Basic Life Support as per AHA/Red Cross standards.
• RN - Licensed Registered Nurse in PA.
• Shift/Hours: Full Time/Days - 36 hours weekly (7a-7p)
• Bachelor’s BSN preferred.
