Qualifications

• Knowledge of patient assessment, care planning, and discharge needs.
• Knowledge of health information documentation and accurate record-keeping.

Responsibilities

• Interacts with co-workers, patients, their family/designated care partner(s), and staff.
• Integrates evidence-based practice and clinical expertise for delivery of care.
• Partners with patients and families to identify learning needs and provide education.
• Minimizes risk of harm by following nursing practices and protocols.
• Uses information technology to communicate, manage knowledge, mitigate error, and support ethical decisions.
• Monitors outcomes of care processes and designs improvements for quality and safety.
• Works within nursing and inter-professional teams with open communication and shared decision-making.
• Recognizes patient and family preferences in safe and coordinated care.
• Creates belonging and inclusion, with problem solving and conflict resolution.
• Seeks opportunities for professional growth and acts as preceptor where appropriate.
• Participates in hospital and practice committees focusing on safety and quality.
