Job summary

Northwell Health is seeking a Registered Nurse to perform age-specific care planning for designated patient groups using the nursing process. You will collaborate with physicians and the health team to coordinate procedures and treatments and to direct the patient care unit team.

The role requires leadership, clinical judgment, and the ability to direct and delegate activities while ensuring patient safety and quality outcomes throughout the care continuum.

Qualifications

• Graduate from an accredited School of Nursing; Bachelor’s Degree in Nursing preferred.
• Must enroll in BSN within 2 years and obtain BSN within 5 years of job entry date.
• Current License to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed.

Responsibilities

• Conducts patient interview, assesses health status across GI, cardiovascular, respiratory, renal and neurological health; determines mobility, sensory deficits, prostheses use, and skin condition and adjusts plan of care.
• Determines coping mechanisms, knowledge level, cultural requirements to reduce anxiety and ensure patient satisfaction; documents care plan and facilitates support groups and social services.
• Develops patient outcome statements and goals; coordinates care activities; documents and implements the plan; ensures safety and discharge planning.
