Job summary

Responsibilities

Assess patient physical, psychological, social, spiritual, educational, developmental, cultural, and discharge planning needs.
Formulate and revise the plan of care including discharge planning with patient/family input.
Implement the plan of care with supervision and coordination of the health care team.
Evaluate patient progress toward goals and outcomes with the team.
Document assessments, planning, implementation and evaluation in the patient record in a timely and accurate manner.
Provide care based on best evidence and participate in improvement activities within clinical practice.
