Ideal candidates hold an active RN license, provide safe medication administration, wound care, and participate in center surveys. You will contribute to continuity of care and ensure residents receive compassionate, high-quality services daily.

Qualifications

• Must hold an active RN license.
• Experience delivering direct patient care.
• Ability to work with CNAs and PCAs to coordinate care.

Responsibilities

• Review and communicate with CNAs and PCAs to coordinate daily tasks.
• Administer medications as ordered by physicians.
• Provide wound care and monitor skin integrity.
• Record and report on resident status and progress for continuity of care.
• Communicate with residents’ families regarding orders and conditions.
• Participate in center surveys and inspections.
• Assist in discharge planning and resident transfers.
• Document care using electronic health records clearly.
