BrightStar Care of Hudson and St Croix Valley is seeking a compassionate Registered Nurse Case Manager to deliver in‑home skilled nursing care for seniors and others in St. Croix and Washington counties. You will develop and update care plans in collaboration with physicians and the care team.

The role offers a part‑time schedule (3 days/wk, Monday–Friday with possible weekends/evenings). A company car is provided, plus a benefits package and professional development opportunities.

Qualifications

• RN license in good standing (MN and WI).
• CPR certification; valid TB test or chest X-ray.
• 1+ year in health care facility or home care; home health exp preferred.

Responsibilities

• Assess client needs and eligibility for home care.
• Plan, implement, and update care plans with the team.
• Provide direct skilled nursing care in clients' homes.
• Manage medications and reconcile where needed.
• Monitor client status and report changes promptly.
• Educate families and caregivers on care plans and safety.

Responsibilities

• Conducting initial and ongoing assessments of clients to determine their care needs.
• Developing, implementing, and updating client care plans in collaboration with physicians and the home health team.
• Providing direct skilled nursing care to clients of varying ages and acuity levels.Coordinating care with families, caregivers, and other healthcare providers.
• Medication Management: Ensuring proper medication administration and reconciliation.
• Client Monitoring: Regularly monitoring clients' conditions and reporting any changes to the appropriate personnel.
• Providing education and support to clients, families, and caregivers.
• Ensuring accurate and timely documentation of client care and progress.
• Ensuring the safety needs of clients are addressed through home environment assessments and individualized abuse prevention plans.
• Providing clinical oversight, delegation, and leadership to other licensed and unlicensed personnel.
• Documentation: Maintaining accurate and up-to-date client records.
• Compliance: Adhering to all relevant regulations, including OSHA and HIPAA.
• Managing your schedule effectively and participating in the on-call phone rotation on occasion.

Qualifications

• Currently licensed and unrestricted as a Registered Nurse (RN) in the State of Minnesota and Wisconsin.
• Certified in CPR and have a negative TB skin test or chest x-ray.
• Minimum of one (1) year of experience in a health care facility or home care setting. Home Health Experience: Two or more years of clinical experience, with one year in a home health agency, primary care clinic, or health facility, is preferred.
• Driving: A valid driver's license and reliable transportation are necessary.
• Demonstrates critical thinking skills and the ability to make appropriate clinical judgments.
• Excellent written and oral communication skills.
• Proficiency in using computer systems and electronic charting is essential.
