Job summary

Park Nicollet is seeking a Registered Nurse to join our Home Care team. The RN assesses patient care needs, provides skilled nursing care and teaching in the patient's home, and coordinates resources to develop a comprehensive plan of care.

The position supports admission, discharge, billing, referrals, and bereavement processes, with a typical schedule of Monday–Friday and rotation every 6–7 weekends, plus on/call occasionally.

Qualifications

• Must have work experience as a RN for at least two years.
• Current MN Registered Nursing License is required.
• BLS certification for health professionals through the American Heart Association or Red Cross required and maintained (online not accepted). Homecare only.
• Demonstrates knowledge of age-related growth and development principles to provide appropriate service and assure safety of patients.
• Proficiency with basic office devices and electronic medical records.

Responsibilities

• Improve or maintain patient health status and prepare patients and families for self-directed and/or assisted care.

Position Summary:

The Home Care registered nurse assesses and evaluates patient care needs, provides skilled nursing care and teaching to patients and families in their home or community - based upon community standards of practice.

The nurse is responsible to improve or maintain patient’s health status, provide terminal illness care, and to prepare patients and their families for self-directed and/or assisted care in their community setting.

The nurse coordinates medical, social, and financial resources to develop a comprehensive plan of care. The nurse supports the following community care processes: admission, discharge, billing, referrals, bereavement, and care delivery.

Territory:

Central (ex: St. Louis Park, Edina, Minneapolis)

Work Schedule:

Monday - Friday 8:00-4:30pm

Every 6-7th weekend rotation

1-2 Holidays per year.

Occasional on/call requirements

Required Qualifications:

• Education, Experience or Equivalent Combination:
  • Work experience as a RN for at least two years
• Licensure/ Registration/ Certification:
  • Current MN Registered Nursing License
  • Current BLS certification for health professionals through the American Heart Association or Red Cross requiredand maintained. On-line BLS certification is not acceptable. Homecare only
• Knowledge, Skills, and Abilities:
  • Demonstrates knowledge of age-related growth and development principles necessary to provide appropriate service and assure safety of patients.
  • Proven proficiency in the operation of the following: Personal Computer, Telephone, Fax Machine, Photocopier, Printer, and electronic medical records.

Preferred Qualifications:

• Experience in home care and or hospice desirable.
• Must be able to function independent with minimal supervision.
• High level of sensitivity and assessment skills

Demonstrates initiative, judgment, critical thinking, and the ability to problem solve.
