Park Nicollet is seeking a Registered Nurse to join our Home Care team. The RN assesses patient care needs, provides skilled nursing care and teaching in the patient's home, and coordinates resources to develop a comprehensive plan of care.
The position supports admission, discharge, billing, referrals, and bereavement processes, with a typical schedule of Monday–Friday and rotation every 6–7 weekends, plus on/call occasionally.

The Home Care registered nurse assesses and evaluates patient care needs, provides skilled nursing care and teaching to patients and families in their home or community - based upon community standards of practice.
The nurse is responsible to improve or maintain patient’s health status, provide terminal illness care, and to prepare patients and their families for self-directed and/or assisted care in their community setting.
The nurse coordinates medical, social, and financial resources to develop a comprehensive plan of care. The nurse supports the following community care processes: admission, discharge, billing, referrals, bereavement, and care delivery.
Central (ex: St. Louis Park, Edina, Minneapolis)
Monday - Friday 8:00-4:30pm
Every 6-7th weekend rotation
1-2 Holidays per year.
Occasional on/call requirements
Demonstrates initiative, judgment, critical thinking, and the ability to problem solve.
