Perham Living - Perham, MN 56573. Position type: Casual/PRN. Salary range: $26.99 – $57.64 per hour.

Shift: Day 8:00am to 4:30pm.

Registered Nurse or Licensed Practical Nurse Home Health | Position Details

The RN or LPN case manager provides skilled nursing care, patient education, and care coordination in collaboration with the healthcare team to support optimal patient outcomes.

• Assess patient and family needs and develop individualized care plans.
• Deliver skilled nursing care and health teaching.
• Monitor patient condition and report significant changes.
• Maintain accurate documentation of visits and care provided.
• Coordinate care with physicians and interdisciplinary team.
• Supervise Home Health Aides and support team collaboration.
• Participate in case coordination, quality improvement, and staff support.
• Assist with staff training, onboarding, and performance evaluation.
• Perform additional duties and leadership responsibilities as assigned.

What You Offer

• Current RN or LPN license in Minnesota.
• Valid driver’s license and auto insurance; ability to travel in all weather conditions.
• Strong clinical, communication, and organizational skills.
• Ability to work independently and maintain professionalism and confidentiality.
• Communicate effectively in English, verbally and in writing.
• Pass a background study through the Minnesota Department of Human Services.
• Patient‑focused care and strong clinical judgment.

Work Schedule

Day Shift–8am to 4:30pm. Casual or roster status.

Compensation

Registered Nurse: $41.82 – $57.64 DOE. Licensed Practical Nurse: $26.99 – $37.21 DOE.
