Responsibilities:
- Plan and provide professional nursing services and standards of practice
- Collaborate, communicate, and facilitate optimal care through computerized and/or digital platforms
- Assess, plan, implement, coordinate, monitor, and evaluate patients’ plans of care from admission to discharge
- Use clinical knowledge, critical thinking, case management, and adult education principles to support plans of care
- Ensure seamless, effective, and efficient transitions of care across the continuum
- Monitor department-specific quality metrics and ensure complete and accurate patient-record documentation
- Serve as mentor, clinician, educator, consultant/collaborator, and navigator
Requirements
- Baccalaureate Degree in the Science of Nursing (BSN) from an accredited school of nursing
- Valid RN licensure authorized in the applicable state(s) of practice/employment
- Valid driver’s license where required by assignment
- Specialty credentialing and educational degree according to clinical nursing practice specialty area
- Minimum five (5) years of RN experience, including adult health condition management
- Electronic Medical Record (EMR) experience
