Position Overview

To support our mission, the Registered Nurse (Oncology Nurse Navigator) will provide nursing and case management services to patients and families, coordinating care across the full continuum of health needs. The nurse will assess, plan, monitor, and evaluate patient care to ensure optimal outcomes, performing assessments, direct and indirect patient care, and documentation.

Key Responsibilities

• Provide disease management, understanding oncology treatments, chemotherapy, and side effects; monitor and address symptoms; educate patients and families about their disease, treatment plans, and quality of life issues; document assessments and interventions promptly in the electronic medical record; collaborate with the healthcare team.
• Effectively triage patient issues in person or by phone; intervene using orders or consultations; address health concerns in cooperation with clinical team members.
• Facilitate communication among physicians, patients, families, and external agencies; provide patient education and document in EHR.
• Perform tasks in accordance with policies, regulations, and best practices, including central line care, patient education, procedures, and medication administration.
• Contribute to high-quality patient care, satisfaction, and team coverage to ensure efficient service delivery.
• Work onsite Monday through Friday, 8:30 am to 5:00 pm, at two locations: Zion on Mondays and Tuesdays, Morton Grove Wednesday through Friday.

Qualifications

• Current Illinois RN license
• BSN or higher preferred
• 3-5 years clinical nursing experience, preferably in surgery and wound care
• Basic Life Support (BLS) certification from the American Heart Association
• Oncology Certified Nurse (OCN) and Bio/Chemotherapy certifications preferred
