Position Overview

To support our mission, the Oncology Nurse Navigator provides nursing and case management services to patients and families, coordinating care across the full continuum. The Nurse demonstrates the ability to assess, plan, monitor, and evaluate patient care, ensuring optimal clinical outcomes. Responsibilities include patient assessment, direct and indirect nursing care, and collaboration with the healthcare team.

Key Responsibilities

  1. Provide disease management with knowledge of oncology, chemotherapy, side effects, and symptom monitoring. Educate patients and families about their disease, treatment plans, and quality of life issues. Document assessments and interventions promptly in the electronic medical record. Support the provider team in managing patient care.
  2. Triages patient medical issues efficiently, either in person or by phone, intervening appropriately and consulting with the interdisciplinary team as needed.
  3. Facilitates communication among physicians, patients, families, and healthcare team members, including external agencies. Provides patient education and documents it in the EHR.
  4. Performs tasks per facility policies, procedures, regulatory requirements, and nursing best practices, such as central line care, patient education, procedure assistance, and medication administration.
  5. Contributes to high-quality patient care, satisfaction, and retention. Works collaboratively, covering for team members to ensure continuous service delivery.
  6. Must be able to work onsite M-F, 8:30 am - 5:00 pm, at two locations: Zion on Mondays and Tuesdays, and Morton Grove on Wednesdays through Fridays.

Qualifications

  • Current IL RN license
  • Bachelor of Science in Nursing (BSN) or higher preferred
  • 3-5 years of clinical nursing experience; surgery and wound care experience preferred
  • Basic Life Support (BLS) certification through the American Heart Association
  • Oncology Certified Nurse (OCN) and Bio/Chemotherapy certifications preferred
