Responsibilities

• Provides office-based nursing care in collaboration with provider.
• Assesses physical and psychosocial needs and plans of care; ensures continuity of care.
• Demonstrates independent nursing actions based on assessment and problem identification.
• Triages telephone calls and initiates appropriate nursing interventions.
• Assists patients and families with concern and empathy; maintains confidentiality.
